Current Monarch Students
Please complete the Transcript Request Form located in the Post Grad Center

ALUMNI (past Monarch Students) Transcripts

All transcript requests must be made in writing either in person, by mail, or by email
​Phone requests can not be fulfilled. 
(Please note: these requests must be made by the student - parents/family members can not request the transcript per The Family Educational Rights and Privacy Act (FERPA))

Provide payment of $2.00 per transcript requested. Transcript will not be mailed until payment has been received.

Complete the pink Alumni Transcript half-sheet found in the counseling office

Include the following information for all requests:
- Your name at time of graduation -
- Year you graduated -
- Date of birth -
- Complete address of where to send the transcript - including the name of the institution -
Official Transcripts can not be faxed

Upon receipt of $2 per transcript, the transcript will be sent
